Lifeplan Education Investment Fund

Additional Deposit Form Use to top up any existing policies. The form should be posted to Lifeplan with a cheque or faxed through if the funds are being deposited by BPay.
EasyClaim Form

Use to make a claim for an Education Savings Plan.

When claiming for education expenses please ensure that the EasyClaim form is sent together with documentation supporting the relevant expense being claimed (eg school invoice). Supporting documentation is only required for expenses up to the amount of education benefits being claimed.

Living Away From Home Declaration Form

Use to make a claim for the 'living allowance' associated with the claims process.

The living allowance is an education benefit that is only available to students living away from home. To claim this amount please complete this form and send to Lifeplan with a completed EasyClaim form.

Plan Beneficiary Nomination Form

Use to alter or nominate a beneficiary. Clients can nominate or alter beneficiary details at any time by completing the attached form. 

Plan Guardian Nomination Form

Use to nominate or change Plan Guardian details.

The Plan Guardian is an estate planning feature and it provides clients with a greater certainty about who will look after the plan arrangements if they were to die or become intellectually disabled.

Student Authority Form Use to provide a nominated student over 16 years of age authority to claim for education related expenses.
Student Nomination Form Use to replace your current nominated student in your plan with another student.

 

Switch Form

Use to switch between funds in an investment.
