Forms for Managed Funds Description
Change of Details Form

Use this form to update your contact details, income distribution arrangement, bank account details or account name.                            

Tax File Number Form 

 Use this form if you wish to provide your Tax File Number (TFN) for your investment.  
Adviser Service Fee Form Use this form to set up a new arrangement to pay your financial adviser a service fee directly from you account, or to change an existing adviser service fee arrangement.
Investor Details Form Use this form when a transfer is to be completed for a closed fund (including Property Syndicate No. 1, Property Syndicate No. 2, Property Syndicate No. 3 and Property Securities Fund).
Generic Withdrawal Request Form*       

Use this form to request a withdrawal from a managed fund*.

Please note - This form is not applicable to withdrawals from the Mortgage Income Trust, High Yield Mortgage Trust, Healthcare Property Trust or Retail Property Fund. Withdrawal information for these products is available through Find a Fund.

Member Alteration Advice

Use this form to update your contact details, financial adviser, regular contribution details, distribution details or to change nominated beneficiaries (Investment Bonds only).

Additional Investment Direct Debit form

 Use this form when making an additional investment (Investment Bonds only).
Memorandum of Transfer Use this form to transfer assignment of an investment bond or funeral bond.
Investment Bond Withdrawal Form

Use this form to request a full or partial withdrawal from an investment bond.

Investment Bond Switch/Extension Form Use this form to switch investment units or extend the term of the policy for an
investment bond.
Maturity Claim Form Use this form to make a claim from your investment bond at maturity.
Bonds Statutory Declaration Form Use this form when a Bond Certificate has been lost.
Investment Bond Death Claim Discharge Form Use this form to claim funds when the life insured for an investment bond has
passed away.
Funeral Bond Death Claim Discharge Form Use this form to claim funds when the life insured of a funeral bond has passed away.

Death Claim Statutory Declaration This form is for use by Funeral Directors wishing to claim a funeral bond.
Funeral Directors Statutory Declaration Use this form when a Bond Certificate has been lost.

 

 

Ordering Product Disclosure Statements

Product Disclosure Statements (PDS) are available to download and print through the Find a Fund section of the Australian Unity Investments (AUI) website.  

If you wish to order hard-copies of AUI Product Disclosure Statements, please download the Product Disclosure Statement Order Form and fax your completed form to (03) 8682 5057.
