Lifeplan accepts faxed and scanned applications (Adviser Use Only)
Lifeplan accepts faxed and scanned documents (this includes applications, claims etc). Scanned documents can be emailed to firstname.lastname@example.org or faxed to 1800 804 890. Please note that we can only process an application after we receive the initial contribution.
